New Clients

We  DO NOT offer free consultation appointments. If you would like to see one of our therapists, you must complete the intake paperwork prior to scheduling an appointment.  

Prior to your first appointment, you will receive an email from CAGE Counseling Services, LLC., with our practice forms. The paperwork should take around 15-20 minutes and we ask that you complete the paperwork within 48 hours of receiving the email. 

Once we receive confirmation that you have completed your credit card on file and intake forms, you will then receive an email or text message to schedule an appointment.  

Currently, we are unable to schedule intake appointments until the necessary paperwork is completed and a card is placed on file. We kindly request that you complete the paperwork within three business days of receiving it.

Our agency uses a third party billing partner, Alma or Headway (depending upon your insurance company), to manage insurance and billing and they have required forms to complete and a credit card MUST be on file to receive services. If your paperwork (CAGE Counseling Intake and/or Alma/Headway) and credit card on file is NOT completed and your insurance cannot be verified, an intake appointment will NOT be scheduled.


The first 1-2 sessions will involve a comprehensive assessment of your needs. By the end of the assessment, your therapist will be able to offer you some initial impressions of what our work might include. At that point, your therapist will discuss your initial treatment goals. You should evaluate this information and make your own assessment about whether you feel comfortable working with him/her. If you have questions about his/her procedures, you should feel free to initiate a discussion with him/her whenever they arise. If your doubts persist, your therapist will be happy to help you set up a meeting with another mental health professional for a second opinion. 

For child/adolescent clients:  

The intake appointment will be conducted in three parts. Your therapist will first meet with the parent(s) and child together, then the parent(s), and lastly he/she will meet with the child. This ensures the therapist completes an accurate assessment of history of issues and description of current issues and complete a treatment plan to meet the child's needs. We ask that if your child is over the age of 13, both the youth and parents fill out the intake forms.

In case of custody situations, you will need to provide a copy of the custody agreement prior to starting services, and both parents will need to sign consent forms. Please fill out and submit all forms at least 1 day prior to your first appointment. If our office does not receive a copy of the custody agreement at least 1 day prior, your appointment will be cancelled and rescheduled once the agreement is received. 


Credit Card on File Requirements


To enhance the efficiency of our services and ensure seamless transactions, we require that a valid credit card be kept on file for billing purposes. Your financial information will be securely stored and used exclusively for authorized transactions related to our services. Should your card experience a decline or expire, please ensure timely updating before your next scheduled appointment to prevent any disruption. Failure to update your card promptly may result in the cancellation of your appointment until the necessary information is provided.
